USPS Form 1583 is required for us to act as an agent on your behalf. We must have the signed and notarized form on file before we can ship or receive your mail. The form may be emailed or faxed, but it must be legible.
Make sure to fill out all the boxes indicated in the interactive forms, sign and date the form, have it notarized (in English or local language), and send us copies of two forms of ID. Notaries can be found at US Consulates, embassies, as well as many other locations (often listed on Google searches). In some cases we can accept other forms of verification, but please check with us first. This is because the Post Office in the US can be very strict about what they will accept.
