USPS Form 1583 Instructions

SPS Form 1583 is required for us to act as an agent on your behalf. We must have the original, notarized form on file before we can ship or receive your mail. No copied, faxed, or e-mailed forms can be accepted.

Make sure to fill out all the boxes indicated in the interactive forms, sign and date the form, have it notarized (in English or local language), and send us copies of two forms of ID. Notaries can be found at US Consulates, embassies, as well as many other locations (often listed on Google searches). If a notary is not available, we will accept forms that are stamped by an official seal, such as that of an attorney.

Acceptable forms of ID when visiting a notary include:
            Valid driver's license or state non-driver's identification card
            Armed forces, government, university or recognized corporate identification card
            Passport or alien registration card or certificate of naturalization
            Current lease, mortgage or Deed of Trust
            Voter or vehicle registration card
            Home or vehicle insurance policy
