USPS Form 1583 Instructions
SPS Form 1583 is required for us to act as an agent on your behalf. We must have the signed and notarized form on file before we can ship or receive your mail. The form may be emailed or faxed, but it must be legible.
Make sure to fill out all the boxes indicated in the interactive forms, sign and date the form, have it notarized (in English or local language), and send us copies of two forms of ID. Notaries can be found at US Consulates, embassies, as well as many other locations (often listed on Google searches). If a notary is not available, we will accept forms that are stamped by an official seal, such as that of an attorney. You may also choose not to have your 1583 notarized, however you must then send a signed passport copy as your third form of identification.
